Dear Providers,

Crystal Coast Wellness and Performance has three convenient locations in Eastern North Carolina to serve your patients. Our main office is located in Morehead City, we also have offices in Cedar Point and New Bern. We have a dynamic team of Registered Dietitians that are able to take on new patients within 1-2 weeks of referral. Our providers are in-network with most major insurance plans. We provide nutrition counseling using a structured program approach that is tailored to each individual patient.

Our established programs are: 

• Weight Management

• Diabetes/Blood Sugar Management 

• Gestational Diabetes

• Pediatrics

• Pre/Post Bariatric Surgery 

• Programs coming soon: Cardiac Health, Sports Performance, and Women's Health

Our Registered Dietitians also council on cardiovascular disease, gastrointestinal disorders, and kidney disease. Individual, couple, and group sessions are available.  

Our providers are in network with most major insurance plans, including Tricare and Medicaid; therefore nutritional visits may be covered with little to no cost to your patients. Telehealth options are available. 

To refer your patients for medical nutrition therapy, fax patient information and a request for a nutrition assessment to our fax number (252) 917-8441. While we do not require a special fax cover form, as most EMS systems have one built into them, we have attached one in the event you prefer a specific form from us.
